How to use these forms

All forms must be completed using Adobe Acrobat.

IMPORTANT Apple and Mac users:

If you are using a Mac OS (Apple computer) please open and complete these fillable PDFs using Adobe Acrobat, not Apple Preview.

Apple Preview usually opens by default on Apple computers, and if completed in Apple Preview your computer will save it in a format that cannot be opened by Student Services.

Forms can be emailed to reghelp@ecuad.ca for processing. We encourage students to email forms to minimize paper waste.

OneCard ID - Replacement Forms

If you have lost, damaged or misplaced your OneCard please see the steps below for ordering a replacement for $15:

Step 1: Please check at the Information (Reception) desk on campus first to see if anyone has turned in your lost OneCard

Step 2: If no card has turned up, please email onecard@ecuad.ca with the following information included:

· Student Number

· Full name

· Your Program/Major

· Whether you are a Domestic or International student

All OneCards are printed weekly on Fridays and are ready for pickup on Friday afternoon after 2 PM. You will receive an email in the afternoon when your card is ready for pickup at Student Services.

All students will be asked to present government issued ID before their card can be issued and new international students will be asked to present their study permit. For returning international students requesting a replacement card please have your BC MSP or your current health insurance coverage available.

Change of Address Forms

Current students can update their addresses through myEC.

Verification of Enrolment Form

Students who require proof of their enrolment may complete the form below and email it to reghelp@ecuad.ca. Please allow 3-5 business days for processing. We require the student's signed authorization to complete the request.

Declare a Minor Form

Students who wish to declare a minor must fill out the following form and email it to an Academic Advisor. More information on Minors is available here.

Information Release Authorization Form

All students' personal information and records are protected by the Freedom of Information and Protection of Privacy Act, Emily Carr University Policy 9.6, therefore Emily Carr cannot assist or give student record access to a third party without written permission from the student.

An information release form can be filled out by the student and provided to the school directly by the student via their Emily Carr email address. The permission will be kept on file in Student Services for the duration of the student's studies unless otherwise notified in writing.

Leave of Absence Form

Undergraduate students may apply for a leave of absence from the university. A leave may be granted by the Registrar for up to one year, in which case the student will be guaranteed a space in their program when they return.

Students leaving the university for more than one year must reapply for admission. If a student needs to leave mid-semester a withdrawal from ALL courses form will also be required. Leave of Absence forms and Withdrawal forms should be emailed to aadvising@ecuad.ca.

Graduate students should contact the Graduate Studies office for information on leave of absence policies and procedures.

Letter of Permission

Students may request a Letter of Permission (LOP) for studies they plan to do at other post-secondary institutions for transfer back to Emily Carr. To guarantee that the coursework will be accepted for transfer, the student must request approval before they register for the course(s) and allow four to six weeks for processing. Please contact transfercredit@ecuad.ca for more information.

Withdrawal Procedures and Forms

In order to withdraw from a course, or from the university, undergraduate students must email an official withdrawal form to reghelp@ecuad.ca. More information on withdrawal policies is available here.

Graduate students should contact the Graduate Studies office for information on withdrawal policies and procedures.
