Health Care Transition

This group project was aimed to help youth with chronic conditions who are transitioning from youth to adult medical care. We focused on educating all youth in high school about the importance of personal health in turn to create a progressive environment for the youth with chronic health conditions. The outcome is an insert outline for teachers who teach Planning 10 in high schools. The content of the insert includes general guides, strategies, in-class role playing game, and grading forms.
