Continuing Studies Certificate Request Form

Please fill out and submit this form once you have completed your certificate program’s requirements.

Certificates are processed and printed twice per year. Once printed, it will be mailed to the specified mailing address you have provided below. Please allow up to 2 weeks for mailing time.

Submit this form by the following deadlines (late forms will not be processed until the next production cycle): 

Submission Deadline | Mailed (subject to change)

• Third Thursday in May for mailing mid-June
• Third Thursday in November for mailing mid-December
