This project aims to help youth with chronic conditions who are in transition from children to adult medical care. We work with clients from the British Columbia Children’s Hospital to generate an effective solution for this problem.

Our group focuses on educating all youth in high school about the importance of personal health in turn to create a progressive environment for the youth with special health conditions. The outcome is an insert outline for teachers who teach Planning 10 in high schools. The content of the insert includes general guides, strategies, in-class role playing game, and grading forms.
